Transitions Senior Program
The Transitions Senior Program is an outpatient program intended to serve adults who are 65 or older who have emotional concerns that interfere with quality of life such as:
- Loneliness
- Withdrawal
- Isolation
- Recent life situation changes - physical, mental, social or emotional
- Grief Issues
- Depressed mood
Program Hours: 10 am - 3 pm, Monday - Friday.
- Lunch is provided daily.
- Transportation is available free within a 25-mile radius of The Transitions Senior Program facility
- Patient's return home at the end of the day

Assessment: Those that are referred to the program will have a complete psychosocial assessment to ensure that the program addresses their needs. Assessment includes discussion of current problems, observable changes in daily activities, medical/emotional problems and issues that might be of concern to the patient. Following the assessment, a personalized treatment plan and schedule is established.
Day Program Activities: Some of the activities and therapies that would make up a typical program day include:
- Cognitive Therapy
- Assertiveness Training
- Grief Management
- Anger Management
- Problem Solving
- Coping Skills
- Feeling and Expression of Feelings
- Leisure Planning
- Relaxation
- Relapse Prevention
