How to see us, your first visit, and where we are

Scheduling an Initial Consultation

To schedule a first visit with a physician of the Division of Vascular Surgery and Endovascular Therapy at Emory, prospective patients should contact (404) 778-3712. Another scheduling option is to contact Emory HealthConnectionSM at (404) 778-7777 or 1-800-75-EMORY. HealthConnectionSM offers immediate access to information on Emory services and physicians and can assist in making appointments.

Referring physicians should contact Julie Bumgardner, clinical coordinator for the division, at (404) 778-3022.

A financial assessment will be performed prior to confirming an appointment, with the following information being requested:

  • Referral from the primary care physician (if surgery is medically necessary)

  • The treating physician's name, address, and telephone number

  • The patient's name, address, telephone number, date of birth, and social security number

  • Diagnosis and date of diagnosis

  • Insurance information

Pre-Registration Form

Appointments can be expediated by downloading, printing, and completing the PDF of the patient pre-registration form (Adobe Acrobat Reader is needed to open and download PDF files) and bringing it to the first visit. Forms cannot be accepted electronically.

First Visit

All pertinent medical records, including recent radiology studies or films, should be brought to the first appointment. In particular, hard copies of recent angiograms and CT scans can be essential to fully assessing the status of the condition. In coordination with the referring physician, a treatment plan is developed at the initial visit based on the medical needs of each individual patient.

Clinical Locations

The reception desk for the Division of Vascular Surgery and Endovascular Therapy is on the 3rd floor of The Emory Clinic, Building A, where outpatient services are offered. Physicians also provide inpatient care at Emory University Hospital, Atlanta VA Medical Center, and Children's Healthcare of Atlanta at Egleston.
